Enhanced Care Management (ECM) is a new statewide Medi-Cal benefit that provides additional services, at no cost, to qualified individuals with complex needs. Through ECM, Koinonia is able to help Medi-Cal managed care plan members address their clinical and non-clinical needs, promoting long-term health in all areas of their life.

In providing this service, Koinonia’s goal is to coordinate care in a way that is collaborative, interdisciplinary, community-based, preventative, and person-centered.

Our ECM services are provided throughout the state of California.

 

What Services Are Provided Through ECM?

Once it’s determined you qualify, Koinonia will provide you with a dedicated care team. They will coordinate with you and your family’s doctors, mental health providers, specialists, pharmacists, case managers, and social services providers. 

 

Your Koinonia care team can help you:

  • Find specialists and make appointments
  • Better understand and keep track of medications
  • Apply for community and social services you may need (i.e. housing support, child care, or nutritious food)
  • Get follow-up care after you leave the hospital

 

How Much Does It Cost?

Accepting ECM services will not change the Medi-Cal benefits you already have. It will give you extra help to better coordinate your care at no cost to you.
